Time to bring back one of my old faves...

THAT... IS A GOOD QUESTION lmao and not one that I think I had considered before. I'm sure he's got some minor fuckups from being punched so many times in his life (thx akihiko) but most of his issues would have been internal, and likely due to the drugs he was on slowly killing him.
They never explain how in game but my VERY BASIC understanding is that most toxins tend to kill off your liver or kidney because that's where they get all filtered and sorted and shit. (Thankfully in MoM that's mostly sorted do to his life-absorbtion-bullshit spending a while internalizing everything he decayed into restoring his insides and junk, hahaha.)
And one in an interview someone affiliated with the game claims he can't regulate his body temperature and so he's cold all the time hence the coat and hat but that's dumb. It doesn't even make biological sense. I could rant about how fucking stupid it is for entirely too long, so I mostly ignore that it was ever said.
(also Persona Q has scenes that contradict that and games > offhand comments by an unnamed developer on the spot during a fan interview to me, anyway.)
SO TO ACTUALLY ANSWER YOUR QUESTION- none that he really notices. He does tend to fall into patterns of insomnia, and he does have plenty of signs of depression (though he's slowly trying to get out of that pattern) but as for physical issues, he's pretty average!
If he is a little more deaf in one ear, it would be the right, because HEY SHOOTING YOURSELF IN THE HEAD THAT OFTEN IS KIND OF LOUD. But I doubt it would be anything super noticeable.
OKAY WAIT NO I REMEMBERED ONE and i don't feel like editing everything that I already typed so you're getting this stream of consciousness style. AW. YEA.
His fucking back, hahaha. His posture is absolutely godawful, and I headcanon that he's got some mild kyphosis going on, with his spine bending funny. I mean seriously, he's hunched over like constantly.
So it likely sounds like firecrackers when he's getting up in the morning, and tends to be far tenser than it needs to be, all muscles knotting up because of the awkward position, but he mostly just deals and keeps going. Akihiko's gotten onto him about it in MoM, but he pretty much refuses to go see anyone about it, because he thinks it would be a waste of everyone's time.
And there's absolutely no canon evidence or whatever but I wanna say when he was young he trended towards asthma but kind of grew out of it BUT THAT'S ONLY BECAUSE I FIND THE IDEA KIND OF CUTE- though lbr it's likely untrue and therefore it doesn't even make it into headcanon status, just "things that would be cute to think about" status
For Inumuta: Do you ever see him growing more into himself socially? or is he always going to be kind of a wallflower nerd. Does he even want to learn to function in society better?

REI AND COOKING I think it was a mix. When he was younger, his mom would be busy so she'd ask for his help with getting dinner ready, or have him make his own bento for lunch because she didn't have time. At first he'd just follow whatever instructions his mom gave him, but after awhile he started fussing over the way the food was presented, which led him to watching cooking shows and reading books to see how the pros presented their food, which led to him finding more recipes he wanted to try.
And from there it snowballed into Rei cooking a lot and everyone just letting him because they didn't have to do it that way and he's not half bad.
The Cycle of Reiness is have to do thing -> want to do thing more beautifully -> work hard to be really good at thing -> rinse, reipeat

Inumuta's actually kind of goalless, but also working towards things? No greater goal in mind at the moment, other than general "find out about the porter, what it does, why it works, why certain people" now with a dash of "why did it take nonon away" in the mix. Shes' the first of the E4 to arrive and then leave, so of course now he has a more determined interest. Also, money. He's going to try and reason a higher wage with Tony Stark, so he can afford for the KLK cast to live in a nicer housing building together. Of course everyone else would have to pitch in, but he has the best credit out of all of them, so...
Post canon, it's confirmed in the OVA he'll be going to college with Iori. Because he's basically attached to the hip with Iori by the ending of the show.

YEAH GOALS AND... ANNIE... the hard part is she evaluates what it is that is practical here versus anything else, so she mostly sticks with practical things she can theoretically achieve. Getting as familiar as she can with the history, expectations, culture, technology, etc, is what she's been doing, and what she'll still be doing. Where he's looking at the porter, Annie has basically surmised she'll be, well, useless, so her efforts end up focusing on the people and those investigations that her skillset is applicable to - who is here in this world, where did the former heroes go, what is going on behind the scenes, because whatever happens with the porter, all the politics of this world and this country are going to be there regardless. She's used to the political morass, so she's looking at better understanding and piecing together the recent history picture. Leaves all that porter stuff to better minds who can actually work with technology.
Though she does eventually want to move her and Reiner out too to something that's not relying on even more politics for the upkeep (stares at government housing) but she figures that's not really going to be doable until she's out of high school, which is... also why she tested out to her senior year so she graduates this year. #planning INDEPENDENCE matters in a sense, but she's practical, and feasibility factors into a lot of what she does, she will never be the sort to turn down handouts (screw pride, she'd rather eat, that sort of thing)
she's so boring that way sometimes it makes me laugh
then again she's also zero percent invested in figuring out how to get home because lolcrystal princess fuck that noise, the people driven to go back to places that matter will figure it out, she'll see what she can suss out here like also if all the imPorts REALLY went home last time
